Three ways to create + submit your survey design

Another Way to Prepare your Survey

Type or copy + paste your survey in the space provided below. Include your directions to us + your respondents as needed.

Upon submission - You will be asked to register + select a feature plan (this is required for us to process your submission). Prior to your survey being finalized + posted online, you will be provided with the opportunity to see a draft of your survey + amend it as you wish.

Three Design Options - Option 1 - use the self-guided form;
Option 2 - type it on your computer, then copy + paste it below; or
Option 3 - use the Survey Design Toolkit with the Survey Library.

Use the space below to type your survey questions (you may use a word-processor and simply copy/paste your data into this form), enter your personal information and hit the send button. If you need additional space, you may submit this form multiple times with fresh data.

1. Number each survey question

2. Indicate the type of response (e.g., range radio button, check-box, fill-in the blank, etc.) you want for each question. See example.

3. If you have instructions you want your respondents to read as they complete the survey, just type them in and pre-label them as "Instructions for Respondent" See example.

4. If you have instructions for SimplyCount to inform +/or help us as we prepare your survey, please label them as "Instructions for SImplyCount". See example.

While you don't have to select your feature plan immediately, you must select a feature plan before SimplyCount will process your survey design request.
